Registration and Eligibility
The Office of Disability Services recognizes 7 types of disabilities:
- Visually Impaired or Blind
- Hard of Hearing or Deaf
- Learning Disabilities (LD)
- Attention Deficit/Hyperactivity Disorders (ADHD)
- Orthopedic Disabilities
- Mental Health Impairments
- Speech Impairment
- Medical Impairments
Candidate Responsibilities
- You will be required to submit medical or other diagnostic records pertaining to your disability and limitations. You may be required to participate in additional evaluations if documentation is outdated or if requesting accommodations not supported in your current documentation.
- You are expected to make timely and appropriate disclosures and written requests for need modifications, including classroom adaptations, test modifications, tutoring, interpreters, taped or enlarged texts, etc.
- You must demonstrate appropriate and responsible levels of self-help in obtaining and arranging for accommodations or auxiliary aids. This includes applying for funding for specialized support services from the Division of Vocational Rehabilitation or other agencies as needed.
Eligibility
The student should:
- Complete, sign and return the Voluntary Disclosure of a Disability Form
- Review the Documentation Guidelines to the right.
- Review the Disability Services Policies and Procedures and sign the Receipt of Policies and Procedures Form.
Documentation Guidelines

Important Reminder
Disability documentation and accommodation requests should be received at least 4-6 weeks prior to the beginning of the semester in order to assure that services can be provided.
